Clinical Usefulness of Three-Dimensional Reconstruction image using the High Resolution CT of the Temporal Bone in Otology

Abstract

|
Background and Objectives : The intricate anatomy of temporal bone has been difficult in understanding. The goal of this study was to investigate the usefulness of three-dimensional (3D) reconstruction of high-resolution CT in determining the anatomy and understanding the topographic relationship between various important structures in the imaginary surgical view.
Subjects and Method : For 20 ears of 10 patients with various otological diseases, 3D reconstruction images based on data from high-resolution CT of temporal bone were obtained by segmentation, volume-rendering and surface-rendering algorithm on a personal computer. The scanning was carried out in axial plane with technical factors of 140 kVp, 100 mAs, 1-mm slice thickness, and 1 second scan time. A software (VworksTM 4.0, CyberMed Inc, Korea) was used for image processing. We measured the angle of genu of facial nerve, diameter of round and oval window, diameter of Eustachian tube, and length of tympanic segment. We also reconstructed the imaginary surgical view, related to the
surgery of internal auditory canal tumor.
Results: We could demonstrate the 3D display of middle and inner ear structures. The reconstruction of anatomical landmark would help us understand topographic relationship in temporal bone and improve surgeon¡¯s understanding of their spatial relationship.
Conclusion: The 3D reconstruction of temporal bone CT can be useful in demonstrating and thus understanding the anatomical structures of temporal bone.
